Utterwell Care Homes provides community outreach support/service for people with learning disabilities or mental health needs.The services aims to provide rehabilitative support to people who have come into community after prolonged stay in residential, hospital or other institutional services. The very objective is to provide support for a brief period of time to help with orientation and establishing structure and daily routine for the people until the support is no longer required. Community Outreach is also offered as an intervention for early signs of mental ill health or relapse prevention for people recovering from serious and enduring mental illness.
